An application for a document may be submitted by: the person to whom the record relates, their ascendants (parents, grandparents), descendants (children, grandchildren), siblings, spouse, court or other state authority and other persons who demonstrate a legal interest.

Lead time
Within 7 working days of filing the application, if the application is filed with the head of the civil registry office keeping the book in which the record is kept. Within 10 working days of filing the request, if the request is submitted to the head of the civil registry office that does not keep the book in which the record is kept.

Stamp duty: - issuance of an abridged excerpt - 22 zł. - issuance of a full excerpt - 33 zł. - issuance of a certificate on entries in the civil status books or their absence, or on the loss or destruction of the civil status book - 24 zł, - issuance of other certificates - 26 zł - certificate of loss or destruction of civil status book - PLN 24. The fee may be paid directly at the toll booth located in the Żory Town Hall at 9 Rynek Street or to the bank account of the Żory Town Hall at 25 Wojska Polskiego Avenue - ING Bank Śląski o/ Żory 95 1050 1070 1000 0022 1347 7389. Appeal mode
Refusal to issue an excerpt, which is made in the form of a decision, can be appealed against to the Governor of the Silesian Voivodship in Katowice. Refusal to issue a certificate takes place by way of a decision, which is subject to a complaint to the Governor of the Silesian Voivodship in Katowice within 7 days of its delivery.

Civil-status registers may be used to issue complete and abridged copies of civil-status records, certificates on the entries made in the civil-status register or on the absence thereof, certificates on the loss or destruction of a civil-status register, certificates on civil status and other certificates relating to the official confirmation of certain facts, for instance a certificate on a fixed marriage date. A legal interest occurs when the production of a copy of a civil-status record or a certificate has a significant importance for the settlement of a case in court or administrative proceedings, which must be documented by a request to the court or to an administrative authority to produce this document or by the production of a court summons confirmed by the court or by a notification to an administrative authority of pending proceedings.
