The application includes the applicant's data and short information about the deed to be entered (who it refers to, where it was drawn up, etc.) The application form can be downloaded from the website and from the customer service points of each building of the Office: - original copy of a deed drawn up abroad or its photocopy confirmed by the foreign office which issued the document, - official translation of a foreign deed into Polish (made by a consul or a certified translator)

Additional info
The head of the civil-status office will carry out a accurate transfer of the foreign civil-status document without any interference in the spelling of the forenames and surnames of the persons shown in the foreign document. If a person has a civil-status record previously established in Poland, he may, together with the transcription request, ask for the spelling of the data contained in the foreign document to be adapted to the rules governing Polish spelling.
