1. wniosek
2. dokument potwierdzający tytuł prawny do nieruchomości, a w przypadku współwłasności - pisemną zgodę wszystkich współwłaścicieli na ubieganie się o dotację; 3. jeżeli zadanie tego wymaga – kopię pozwolenia wydanego na podstawie przepisów prawa budowlanego, a jeśli przepisy wymagają zgłoszenia, dokument potwierdzający zgłoszenie robót budowlanych do organu administracji, z adnotacją lub zaświadczeniem organu
o braku sprzeciwu do zgłoszenia;
4. kopie dokumentów, o których mowa w przepisach o wyrobach zawierających azbest, potwierdzających złożenie zgłoszeń do właściwych organów o zamiarze przeprowadzenia prac polegających na usuwaniu lub zabezpieczaniu wyrobów zawierających azbest z obiektu, jeżeli wnioskodawca dysponuje tymi dokumentami w terminie złożenia wniosku albo oświadczenie o zamiarze złożenia tych zgłoszeń;
5. oświadczenia, zgodnie z załącznikiem nr 1 do wniosku;
6. oświadczenia, informacje zgodnie z przepisami ustawy z dnia 30 kwietnia 2004 r. o postępowaniu w sprawach dotyczących pomocy publicznej (dotyczy podmiotów prowadzących działalność gospodarczą).

2. Document confirming legal title to the property, and in the case of joint ownership, written consent of all joint owners to apply for the subsidy; 3. If the task requires it, a copy of the permit issued under the provisions of the Building Code, and if the regulations require notification, a document confirming the notification of the construction works to the administrative authority, with an annotation or a certificate from the authority confirming no objection to the notification;
4. Copies of documents referred to in the regulations on asbestos-containing products, confirming the submission of notifications to the competent authorities of the intention to carry out work involving the removal or securing of asbestos-containing products from the facility, if the applicant has these documents by the time of submitting the application, or a declaration of intent to submit these notifications;
5. Declarations, in accordance with Annex 1 to the application;
6. Declarations and information in accordance with the provisions of the Act of 30 April 2004 on the Procedure for Proceedings in Matters Concerning Public Aid (applies to entities conducting business activities).

Legal basis
Resolution No. 233/XVIII/25 of the Żory City Council of 11 September 2025 on establishing the principles for granting targeted subsidies from the budget of the Municipality of Żory to co-finance investment costs related to the implementation of projects involving the modernization of roof coverings or facades of buildings containing asbestos.
Uwagi
Inwestycje do dofinansowania powinny spełniać następujące kryteria:
1. Wnioskodawca musi posiadać tytuł prawny do nieruchomości w Żorach, na której zinwentaryzowano wyroby zawierające azbest.
2. Informacja o obiekcie budowlanym powinna znajdować się w Bazie Azbestowej.
3. Wnioskodawca powinien uzyskać zgodnie z przepisami prawa budowlanego dokumenty uprawniające do rozpoczęcia robót budowlanych, lub dokonać zgłoszenia robót budowlanych, jeżeli są wymagane.
4. Wnioskodawca powierzy wykonanie prac polegających na usuwaniu lub zabezpieczaniu wyrobów zawierających azbest oraz gospodarowanie usuniętym wyrobem zawierającym azbest (stanowiącym odpad) uprawnionym podmiotom.
5. Wykonawca zadania dokona zgłoszeń, o których mowa w przepisach o wyrobach zawierających azbest, o zamiarze rozpoczęcia prac polegających na usuwaniu lub zabezpieczaniu wyrobów zawierających azbest.
6. Wnioskodawca zapewni realizację inwestycji przy kwocie dotacji udzielonej przez Gminę w wysokości 80% kosztów kwalifikowanych zadania, obejmujących demontaż wyrobów zawierających azbest, transport i unieszkodliwianie odpadów zawierających azbest, nie więcej jednak niż 5.000,00 zł.
Comments
Investments eligible for funding must meet the following criteria:
1. The applicant must hold legal title to the property in Żory where asbestos-containing products have been inventoried.
2. Information about the construction must be included in the Asbestos Database.
3. The applicant should obtain documents authorizing the commencement of construction works in accordance with the provisions of the Construction Law, or submit a construction works notification, if required.
4. The applicant will entrust the removal or securing of asbestos-containing products and the management of the removed asbestos-containing product (constituting waste) to authorized entities.
5. The contractor will submit notifications, as specified in the regulations on asbestos-containing products, of the intention to commence work involving the removal or securing of asbestos-containing products.
6. The applicant will ensure the implementation of the investment with a subsidy granted by the Municipality in the amount of 80% of the eligible costs of the project, including the removal of asbestos-containing products, transport, and disposal of asbestos-containing waste, but not exceeding PLN 5,000.00.
