The request for the rectification of a civil status record must be submitted to the head of the civil registry office which established the civil status record to be corrected.

Without undue delay, when the case requires an enquiry, within one month from the moment the required documents are submitted. Correction is carried out in the form of a material and technical action ending with the issue of a full copy of the civil status record.

An application containing the applicant's data and brief information on the deed to be rectified (who it relates to, what data in the deed are to be rectified, etc.) If the civil status record is to be rectified on the basis of a Polish civil status record, there is no need to produce a copy of such a record. If the correction is based on a foreign document, it is necessary to enclose that document together with its official translation into Polish. This document should contain the data subject to correction, state an earlier event and concern the same person or their ascendants (e.g. parents)

Appeal mode
The refusal of rectification shall take the form of an administrative decision which may be appealed against to the Silesian Province Governor.

It is possible to rectify a civil-status record containing data that are inconsistent with those contained in collective records or with other civil-status records, insofar as they mention a previous event and concern the same person or his descendants, or with foreign civil-status documents. If the rectification of a civil-status record is carried out ex officio or at the request of a person other than the person concerned or his legal representative, this person will be informed by the head of the civil-status office of any intention to rectify his civil-status record or of the request for rectification.
