Appeal mode
A party dissatisfied with the issued decision may, within 14 days of its delivery through the Head of the Registry Office in Żory, file an appeal with the Silesian Province Governor in Katowice. The appeal is exempt from stamp duty.

A request to change a forename or surname shall be submitted to the head of the register office of one's choice. Persons residing outside the Republic of Poland may submit, through the intermediary of a consul of the Republic of Poland, a request to change their forename or surname, indicating the head of the register office to which the request should be forwarded. A change of a forename or surname of a minor child takes place upon a written request of the child's legal representative. Such a change requires the consent of the other parent, and if at the moment of the name change the child is over 13 years of age, the child's consent is also required. The consent of the child and the other parent to a change of the child's surname or forename should be given in person before the Head of the Registry Office or in writing with a notarially certified signature.
