Ponadto:
- Do wglądu dokumenty tożsamości osób zamierzających zawrzeć związek małżeński: dowód osobisty lub paszport;
- pisemne zapewnienie (przygotowane przez pracownika USC) o "braku istnienia okoliczności wyłączających zawarcie małżeństwa" złożone przed Kierownikiem USC;
- zezwolenie sądu opiekuńczego na zawarcie małżeństwa przed uzyskaniem wymaganego wieku (wymagany wiek dla mężczyzny i kobiety - 18 lat);
- potwierdzenie dokonania opłaty skarbowej.
Cudzoziemiec zamierzający zawrzeć małżeństwo składa:
1) pisemne zapewnienie (przygotowane przez pracownika USC) o "braku istnienia okoliczności wyłączających zawarcie małżeństwa" złożone przed Kierownikiem USC;
2) odpis aktu urodzenia, a jeżeli pozostawał uprzednio w związku małżeńskim odpis aktu małżeństwa z adnotacją o jego ustaniu, unieważnieniu albo stwierdzeniu jego nieistnienia, albo odpis aktu małżeństwa z dokumentem potwierdzającym jego ustanie lub unieważnienie albo dokumentem potwierdzającym stwierdzenie nieistnienia małżeństwa, jeżeli na podstawie pozostałych składanych dokumentów nie można ustalić danych niezbędnych do sporządzenia aktu małżeństwa;
3) dokument stwierdzający, że zgodnie z właściwym prawem (państwa, którego jest obywatelem) może zawrzeć małżeństwo poza granicami kraju albo prawomocne postanowienie sądu o zwolnieniu cudzoziemca od obowiązku złożenia takiego dokumentu;
4) dokument tożsamości.
Required Documents
An application containing the information of the applicant and the person with whom the applicant intends to marry.
In addition:
- Identification documents of the persons intending to get married: identity card or passport;
- a written statement (prepared by an employee of the Registry Office) on "the lack of circumstances excluding the contracting of marriage" made before the Head of the Registry Office;
- permission from the guardianship court to get married before reaching the required age (the required age for men and women is 18);
- confirmation of payment of stamp duty.
A foreigner who intends to enter into marriage submits
1) a written assurance (prepared by an employee of the Registry Office) that "there are no circumstances excluding marriage" made before the Head of the Registry Office;
2) a copy of birth certificate, and if they were previously married a copy of marriage certificate with an endorsement about its cessation, annulment or non-existence, or a copy of marriage certificate with a document confirming its cessation or annulment or a document confirming non-existence of marriage, if on the basis of other submitted documents data necessary for drawing up a marriage certificate cannot be determined;
3) a document stating that under the applicable law (of the country of which they are citizens) they may enter into marriage outside the country or a final court decision releasing a foreigner from the obligation to submit such a document;
4) an identity document.

Stamp duty: The stamp duty of PLN 84 should be paid to the bank account of the Town Hall in the area where the marriage will take place. If the marriage will take place in Żory, the fee may be paid directly at the stamp duty station located in the Żory Town Hall at 9 Rynek Street or to the bank account of the Żory Town Hall at 25 Wojska Polskiego Avenue - ING Bank Śląski o/ Żory 95 1050 1070 1000 0022 1347 7389. Tryb odwoławczy
Appeal mode
If the parties are notified in writing of the refusal to issue a certificate on the absence of circumstances excluding the conclusion of a religious marriage or to draw up a marriage certificate concluded before a clergyman, the party may request the District Court to decide whether the circumstances presented by the Head of the Registry Office justify the refusal to perform an act.

Persons who intend to get married, in order to obtain a certificate of lack of circumstances excluding the conclusion of a religious marriage, are obliged to submit a written statement of no impediment to marriage. The statement of assurance may be submitted before any head of the Registry Office in Poland and abroad before a Polish consul or public notary. A foreigner who does not speak Polish must submit this statement in the presence of a certified translator. Beneficiaries wishing for their religious marriage to have effect under Polish law must submit the above mentioned certificate to a clergyman. The certificate is issued on the basis of the statements referred to above. The certificate is issued by the head of the registry office to which a person entering into marriage applies. It is valid for a period of 6 months. After expiry of this period, the certificate becomes invalid. The clergyman shall submit to the register office, within 5 days from the moment of marriage, a certificate of marriage and a certificate issued by the head of the register office, on the basis of which the marriage certificate is drawn up immediately.
