The request to complete the civil-status record must be submitted to the head of the civil registry office that established the civil-status record to be completed.
Without undue delay, when the case requires an investigation, up to one month after the submission of the required documents.
An application containing the applicant's data and brief information on the record to be completed (whom it concerns, what data are to be added to the record). If the civil status record is to be completed on the basis of a Polish civil status record, it is not necessary to produce a copy of such a record. If the basis for the supplementation is a foreign document, it is necessary to enclose that document together with its official translation into Polish. The document should contain the data which are to be supplemented, state an earlier event and relate to the same person or to his ascendants (for example, parents).

Refusal to supplement the act takes the form of an administrative decision, which may be appealed against to the Silesian Province Governor in Katowice.

If the completion is carried out by virtue of public office or at the initiative of persons other than the person whose civil status record relates or his legal representative, this person will be informed by the head of the civil registry office of the intention to complete his civil status record or to submit a request for completion.
