The decision to enter or refuse entry in the register of electors is issued within 5 days from the date of the application.

Do wypełnionego wniosku o ujęcie w stałym obwodzie głosowania (tzw. wpisanie do rejestru wyborców) załączyć należy dokument potwierdzający, że wnioskodawca stale zamieszkuje na terenie miasta Żory. Takim dokumentem może być na przykład:

• pierwsza strona zeznania podatkowego PIT za rok ubiegły (strona z danymi osobowymi i adresem zamieszkania),
• zaświadczenie wydane przez Urząd Skarbowy o odprowadzaniu podatku w Żorach,
• dokument potwierdzający tytuł prawny do lokalu, w którym wnioskodawca stale zamieszkuje (np. umowa najmu),
• oświadczenie właściciela lokalu lub innej osoby dysponującej tytułem prawnym potwierdzające, że wnioskodawca stale zamieszkuje pod wskazanym adresem.

W wyjątkowych sytuacjach, np. w przypadku niedysponowania dokumentami potwierdzającymi zamieszkiwanie we wskazanym miejscu, dopuszcza się przeprowadzenie oględzin lokalu celem potwierdzenie stałego zamieszkiwania w deklarowanym miejscu. 

1. Application for registration of the voter in the Electoral Register,

2. A photocopy of a valid document confirming the identity of the applicant,

3. Written declaration containing information necessary to be added to the voter register.

4. Documents for inspection: identity document (ID card, passport, residence card, etc.)

The parties are entitled to lodge a complaint against the decision on refusal to enter the voter register with the District Court in Żory through the Mayor of Żory within 3 days of delivery of the decision.
