Required Documents
In order to report the fact of unauthorised use of personal data, the holder of an ID card must provide a confirmation of a notification of a suspicion of unauthorised use of the notifier's personal data, including the series and number of the ID card, a document confirming the initiation of ex officio actions by this authority or a decision of the President of the Office for Personal Data Protection stating the breach of provisions on personal data protection with regard to security of such data of the ID card holder, including the series and number of the ID card.
