Do 30 dni. W sprawach szczególnie skomplikowanych nie później niż w ciągu 60 dni

Zgłoszenie powinno zawierać (art. 101e ust. 4 ustawy Prawo ochrony środowiska):

1. imię i nazwisko albo nazwę podmiotu zgłaszającego oraz adres jego zamieszkania lub siedziby,

2. wskazanie miejsca, w miarę możliwości poprzez podanie adresu lub numeru działki ewidencyjnej,

3. informacje na temat czasu wystąpienia zanieczyszczenia powierzchni ziemi, w tym, w miarę możliwości, dokumenty uprawdopodobniające, że zgłoszenie dotyczy historycznego zanieczyszczenia powierzchni ziemi,

4. opis stwierdzonej sytuacji wskazującej na występowanie potencjalnego historycznego zanieczyszczenia powierzchni ziemi, dokumentację, która uprawdopodobni jej wystąpienie, w tym, w miarę możliwości, nazwy substancji powodujących ryzyko oraz wyniki badań zanieczyszczenia gleby i ziemi tymi substancjami, wykonanych przez laboratorium, o którym mowa w art. 147a ust. 1 pkt 1 lub ust. 1a ww. ustawy, tj. przez akredytowane laboratorium w rozumieniu ustawy z 30 sierpnia 2002 r. o systemie oceny zgodności lub we własnym laboratorium, pod warunkiem że laboratorium to jest również objęte systemem zarządzania jakością lub jest zapewniony automatyczny pobór prób przy użyciu próbobierni objętej nadzorem metrologicznym.

The notification should include (Article 101e paragraph 4 of the Environmental Protection Law):

1. the name of the notifier and the address of their residence or registered office,

2. an indication of the location, if possible with the address or registered parcel number,

3. Information on the time of occurrence of the surface contamination, including, if possible, documents demonstrating that the notification concerns historical surface contamination,

4. description of the identified situation indicating the occurrence of potential historical land surface contamination, documentation substantiating its probability, including, where possible, the names of the substances causing risk and the results of soil and ground contamination tests with those substances, performed by a laboratory referred to in Art. 147a (1) (1) or (la) of the aforementioned Act, i.e. by an accredited laboratory within the meaning of the Act of August 30, 2002 on the compliance assessment system or by its own laboratory, provided that the laboratory is also covered by the quality management system or automatic sampling is ensured by means of a sampling device under metrological supervision.
